Integrated Wellness Center offers a variety of diagnostic tests, including blood tests, and Bio-Merdian testing

We suggest that each patient call their health insurance company directly to see if Dr. Choi is in network with your individual plan.  Although we can tell you  whether we accept a large company like Blue Cross we cannot tell if your individual plan is in network with Dr. Choi.  We do NOT accept any plans with Medicare, Medicaid, or Tricare.

Please bring a photo ID, your insurance card, and any relevant medical records or referral forms to your appointment at Integrated Wellness Center.

The length of time it takes to receive test results can vary based on the type of test. Your healthcare provider will inform you of the expected time frame for receiving your test results.  Lab results are generally 1-3 days. We will receive your results from the lab and reach out to you about the results.

Patients can call the main line and leave a voicemail message on extension #4, or use the patient portal to get refills and we will get back to you within 24 hours.

We require that you schedule an appointment for lab draws but you're welcome to call the front desk and see if there are available openings for same day walk ins.

We do accept Life Extension.  After you sign up online and pay for your test with them you will receive a test requisition form. Bring this form to our office and we will draw your labs for the $20.00 venipuncture fee. We will then send your labs over to the requested lab designated by you.

We charge $20.00 for venipuncture, which is due at the time of the appointment. Labcorp charges $26.25 for self pay patients venipuncture, in addition to the cost of the labs. We send the test requisition form to Labcorp, or lab of your choice, and they will bill your insurance or you depending on your insurance status.
